First Visit
What Will Happen On My First Visit?
We will complete an in-depth medical and dental history and discuss the concerns you have about your dental and periodontal health. A thorough assessment of the health of your gums and the bone supporting your teeth will be recorded and we will be evaluating the mobility of teeth, spaces where teeth are missing, as well as the overall health of your mouth. This appointment usually takes one hour and will give us time to discuss our findings and any treatment recommendations we may have. We encourage you to bring family members with you if they would like to be present for your consultation time.
Please assist us at the time of your initial visit to the office by providing the following information:
- Any referral slips or x-rays your dentist sent with you
- Please fill out your patient registration and medical history and bring this to your appointment. You can fill these out online and submit them by email or print them to fill out and bring to your appointment.
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to your examination that requires premedication with antibiotics (e.g. artificial heart valves or artificial joints, heart murmurs requiring pre-medication, previous infective endocarditis, congenital heart disease, cardic transplantation recipients). The antibiotic needs to be taken one hour prior to your dental appointment, please contact your doctor or dentist if you are in need of a prescription.
