Financial Policy
Financial Information
Dr. Collins and his staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy. The initial exam and consultation fee is due the date of your initial appointment regardless of whether you have dental insurance or not. We do not file medical insurance, Medicare or Medicaid.
Emergency/Single-Visit Payments
For single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders, and credit card payment (MasterCard, Visa, and Discover). If you have dental insurance the claim will be mailed to the insurance company and they will mail payment directly to you for any benefits.
Multiple Visit Payment Options
Option 1: Full Payment at Time of Service – For patients without dental insurance.
Option 2: Patients with dental insurance – Pre-treatment estimates are obtained from your insurance company prior to treatment and you are responsible for the portion insurance does not cover at the time treatment is rendered. Please see the insurance information page for more details.
Option 3: Outside Financing – For those who would prefer an extended payment plan for fees over $1.000.00, we accept CareCredit plans for those patients that qualify. You can apply for this financing by going to www.carecredit.com and filling out the online application prior to your treatment appointment.
For your convenience, payments may be made with cash, personal check, Discover, Visa, or MasterCard. If you have questions about the financial policy, please ask our staff. They are well informed and can be reached by phone at 806-356-9448 or by fax at 806-356-7159. Our office location is 10 Care Circle Suite B. Please call if you have any questions or concerns regarding your initial visit.
